Clitoral Glans Reduction

Clitoromegaly, or enlargement of the clitoris, is becoming more common because of the widespread use of testosterone therapy.  Some women taking Testosterone therapy, especially in higher doses, can develop clitoromegaly.  Clitoromegaly can also occur in women who are genetically predisposed to develop an enlarged clitoris or due to medical conditions.

When clitoromegaly occurs, the clitoral body, which is the erectile tissue of the clitoris, enlarges.  This is what gives the clitoris a more phallic and drooping appearance.  The clitoral glans, or visible portion of the clitoris, also enlarges and is what people typically notice the most when the area is enlarged.  It is important to note that the clitoral glans does not contain significant erectile tissue.

Clitoral glans reduction is achieved by surgically reducing the visible, non-erectile clitoral glans, and elevating the erectile clitoral body by way of clitoropexy.  Elevation of the clitoris (clitoropexy) reduces the drooping or sagging appearance of the clitoris and clitoral hood. Reduction of the glans reduces the external appearance of the clitoral bulb. The combination of the clitoral glans reduction and clitoropexy along with clitoral hood reduction helps patients achieve the goal of a more natural looking clitoris.

The overall goal of clitoral glans reduction is change in appearance as well as improvement in sexual response following the procedure. Sexual response is multifactorial and other factors may play a role in the overall outcomes. Negative sexual changes post operatively are most often temporary and resolve with time, however, there is a small risk of long standing negative effects on the sexual response.
